Social Care WorkerWorcester22/04/2025 – 2 months initiallyMonday to Friday, 9am – 5.30pm, 37 hrs/week
  • Ensure that the outcome of all work with adults in need of care and support, and with their carers, is to promote their wellbeing
  • Carry out and review Needs Assessments & Care and Support Plans for individuals and their carers
  • Engaging with people to understand the reason for their contact and their presenting needs and to consider options for resolving these at an early stage
  • Ensure opportunities for reablement and independent living are always considered and explored, enhancing individual and family resilience
  • Work with the individual in their local environment supporting the individual to build support within their local community
  • Ensure information and advice is provided
  • Work closely with primary care/community services/hospital teams/voluntary community services
  • Keep and maintain records of work
  • Attend meetings and reviews where necessary
Requirements
  • A professional qualification relevant to the post such as Health and Social Care
  • Experience working with Adults with complex needs
  • Experience in assessments and support planning
  • Adult DBS registered to the update service
