Sanctuary Personnel, an innovative and committed recruitment agency has a new permanent position available for a Social Worker within the Pathway Planning Team to work full time based in County Hall, Worcestershire.

The salary for this permanent Social Worker job is up to £39,186 per annum. Main duties:

  • Work with adults, using a strength based approach, who have complex or fluctuating needs and require intensive support to reduce and manage those risks.
  • Work in partnership and engage other professionals to ensure there is a multi-agency approach which enables the adult and/or their carer to live as independently as possible.
  • May be expected, on occasions, to represent the Authority in court and also may be required to attend as a witness or give evidence.
  • Represent the service, adults and carers’ views at a range of meetings and exchange information, make decisions and review progress.
  • Build to excellent and effective relationships with other agencies and the local community to signpost when appropriate.
  • Gather and analyse from a variety of sources that will provide an informed judgement for the decisions you make.
  • Identify any potential deprivation of liberty and ensure the least restrictive means of care provision prior to instigating legal proceedings.
  • Undertake duty responsibilities in line with the requirements of the team.
  • Maintain accurate records in-line with the Adult Services’ Recording Policy
  • Prepare and participate in reflective practice and supervision.
  • The Social Worker must meet the continuing professional development requirements of the professional regulator and keep abreast of legislative changes.
  • To maintain personal and professional development to meet the changing demands of the job, participate in appropriate training activities and encourage and support staff in their development and training.
  • To undertake other such duties, training and/or hours of work as may be reasonably required and which are consistent with the general level of responsibility of this job.
  • Through meaningful conversations, you will carry out and review assessments and support plan for adults and their carers to build a good life within their community.

Requirements of this role:

  • A social Work Qualification or equivalent.
  • Understanding of relevant legislation.
  • Registered or eligible for registration.
  • This post requires registration with the relevant health professional council/governing body (Social Care England).
