  • Key Responsibilities:
    • Patient Care:
      • Assess, plan, implement, and evaluate individualized care plans for patients.
      • Provide direct nursing care in patients homes or community settings.
      • Ensure high standards of care based on current research and best practices.
    • Clinical Duties:
      • Manage a delegated caseload of patients under the supervision of a senior nurse.
      • Perform initial and risk assessments of patients and provide immediate care as needed.
      • Maintain accurate and confidential patient records.
    • Team Collaboration:
      • Work closely with other healthcare professionals, including GPs, social services, and voluntary agencies, to meet the needs of the local population.
      • Provide clinical advice and support to other staff members.
      • Participate in the training and supervision of junior staff and students.
  • Skills and Qualifications:
    • Registered Nurse with a valid NMC (Nursing and Midwifery Council) registration.
    • Strong clinical skills and the ability to work independently.
    • Excellent communication and interpersonal skills.
    • Basic IT skills for data collection and record-keeping.
