Patient Care:
- Assess, plan, implement, and evaluate individualized care plans for patients.
- Provide direct nursing care in patients homes or community settings.
- Ensure high standards of care based on current research and best practices.
Clinical Duties:
- Manage a delegated caseload of patients under the supervision of a senior nurse.
- Perform initial and risk assessments of patients and provide immediate care as needed.
- Maintain accurate and confidential patient records.
Team Collaboration:
- Work closely with other healthcare professionals, including GPs, social services, and voluntary agencies, to meet the needs of the local population.
- Provide clinical advice and support to other staff members.
- Participate in the training and supervision of junior staff and students
Skills and Qualifications:
- Registered Nurse with a valid NMC (Nursing and Midwifery Council) registration.
- Strong clinical skills and the ability to work independently.
- Excellent communication and interpersonal skills.
- Basic IT skills for data collection and record-keeping.
