An exciting opportunity has arisen for an experienced and motivated Band 6 Community Physiotherapist to join the UCR Team in West Sussex. This role will focus on providing high-quality care to patients in the community, specifically within respiratory and elderly care.

Start Date: ASAP
Hours: Full-time (Part-time considered)
Hourly Rate: £25-£28 depending on experience

As a key member of the team, the successful candidate will be responsible for assessing and managing a diverse caseload, ensuring that patients receive tailored rehabilitation and physiotherapy interventions to promote recovery, independence, and well-being. This is a fantastic opportunity to work within a supportive multidisciplinary team that focuses on delivering patient-centred care in the community.

Key Responsibilities:

  • Deliver physiotherapy services to patients with respiratory and elderly care needs, including home visits.
  • Conduct comprehensive assessments and develop individual treatment plans.
  • Work closely with a multidisciplinary team, including GPs, nurses, and other healthcare professionals.
  • Provide rehabilitation and education to help patients manage their conditions and improve their mobility.
  • Maintain accurate patient records in line with local and national clinical standards.
  • Promote and support self-management strategies for patients.
  • Liaise with other services and agencies to ensure continuity of care.

Essential Requirements:

  • HCPC registered physiotherapist.
  • Proven experience in respiratory and elderly care physiotherapy.
  • Full, valid UK driving license and access to a car for home visits.
  • Strong communication and interpersonal skills, with the ability to work independently and as part of a team.
  • A flexible and compassionate approach to patient care.
  • Ability to manage a caseload effectively and efficiently.

Desirable:

  • Experience working in a community setting.
  • Knowledge of reablement and rehabilitation strategies for older adults.

This position offers an excellent hourly rate between £25-£28, depending on experience, and the opportunity to make a significant impact on the care and well-being of patients in the community. The role is available as full-time, but part-time options will also be considered.
