Job Overview:We are seeking a dedicated Band 6 Occupational Therapist to join our team at Worthing Hospital. In this role, you will be responsible for conducting therapy assessments, developing treatment plans, and managing discharge processes for patients on acute general medicine, frailty wards, and front door teams.

Key Responsibilities:

  • Conduct comprehensive therapy assessments for patients across designated wards.
  • Develop and implement effective treatment and discharge plans tailored to individual patient needs.
  • Collaborate with multidisciplinary teams to ensure seamless patient care and transitions.
  • Monitor and evaluate patient progress, adapting interventions as necessary.
  • Maintain accurate documentation and ensure compliance with best practice standards.

Key Requirements:

  • A recognized qualification in Occupational Therapy.
  • Previous experience in an acute setting, particularly in general medicine or frailty care.
  • Strong assessment and clinical reasoning skills.
  • Excellent communication and teamwork abilities.
  • Registration with the Health and Care Professions Council (HCPC).
