Qualified Social Worker - Older Peoples Services 65+ - Wrexham £41.12We are looking to recruit a Qualified Social Worker - Single Point of Access Team within the Social Care industry in Wrexham. The successful candidate will be working in a well-structured, organised team who are growing their social worker staffing levels. The Qualified Social Worker - Single Point of Access vacancy in Wrexham will be for an initial 3-6 months with potential for extension subject to internal circumstance and your performance.

Responsibilities include;

  • The post is for an initial 3 month period. Adults SPoA are the first point of contact for referrals to Adult Social Care in Wrexham receiving referrals from other professionals including individuals, their friends, carers/families; North Wales Police: Ambulance Service: Fire Service: Doctors; Hospitals District Nurses; Housing etc.. The team make enquiries on all referrals received into Adult SC and determine the most appropriate pathway for them, also considering whether the individual may have needs for care and support or may be an Adult at Risk through initial assessments. The team complete a what Matters conversations working with the individual to identify their personal outcomes and any solutions that can assist them to achieve their wellbeing through the provision of information, advice and/or assistance or signposting to other teams/services

Experience preferred:

  • Post qualifying experience of working as a Qualified Social Worker - Older Peoples 65+ Team within the UK Social Care industry.

Minimum Requirements:

  • Care Council of Wales
  • Enhanced DBS (we can obtain for you)
  • Eligibility to work in the UK
