Wrexham Council are seeking a qualified Social Worker to join their team within the Older People Services. The successful candidate will manage an allocated caseload, undertake assessments, develop care and support plans, and work in partnership with all services to ensure best use of resources. They will also be responsible for ensuring practice and subsequent decisions safeguard the welfare of adults at risk and children at risk of immediate harm.
Key Responsibilities- Provide professional social work services to adults/children, their families and carers by undertaking outcome-focused assessments to identify eligible needs and developing outcome-focused care and support plans.
- Establish, develop and maintain professional relationships through direct casework, supervision, with the aim that the individuals identified needs are met.
- Ensure that all assessments, planning, implementation and reviewing of caseload and decisions and practice are of the expected standard as required by the service.
- Identify, assess and report Adult/Child at risk and undertake safeguarding enquiries for Adult/Child at risk in accordance with The Wales Safeguarding Procedures.
- Promote individuals/ people needing care and support, their families and carers in need of support participation in assessment planning, implementation and reviewing and are offered advocacy when appropriate.
- Social Work Degree or Equivalent Qualification
- Newly qualified candidates will be considered
- Court work desirable but not essential
- Commitment to gain Consolidation Award by the end of first period of registration
- Register and maintain registration as a Social Worker with Social Care Wales
- Current and up to date knowledge of relevant Social Care Legislation for adults and children's services / Education
