About the Role:
As a Learning Disabilities Social Worker at Somerset Council, you will play a pivotal role in empowering individuals to live fulfilling and independent lives. You will work within a supportive and collaborative multi-disciplinary team, providing comprehensive assessments, developing person-centred support plans, and advocating for the needs of your clients.
Key Responsibilities:
- Conduct comprehensive assessments of individuals with learning disabilities, identifying their needs and strengths.
- Develop and implement person-centred support plans, ensuring they are tailored to individual needs and promote independence.
- Work collaboratively within a multi-disciplinary team, including health professionals, therapists, and support staff.
- Build and maintain strong partnerships with other agencies, including Social Care Services, Health Services, Voluntary Agencies, and Education, to ensure seamless service delivery.
- Provide advocacy and support to individuals and their families, ensuring their voices are heard.
- Safeguard vulnerable adults, adhering to all relevant policies and procedures.
- Maintain accurate and up-to-date records, ensuring compliance with data protection regulations.
- Provide crisis intervention and support as needed.
- Contribute to the ongoing development and improvement of services for individuals with learning disabilities.
- Participate in regular supervision and training opportunities.
What We Are Looking For:
- A qualified Social Worker registered with Social Work England.
- Significant and relevant multi-disciplinary Social Care experience within a multi-disciplinary team, specifically related to the needs of individuals with learning disabilities.
- Proven ability to conduct comprehensive assessments and develop person-centred support plans.
- Strong evidence of effective partnership working with other agencies, including Social Care Services, Health Services, Voluntary Agencies, and/or Education.
- In-depth knowledge and understanding of Social Care practice as defined by Social Work England Guidelines, appropriate legislation (e.g., Care Act 2014, Mental Capacity Act 2005), and issues relevant to the client group.
- Excellent communication and interpersonal skills, with the ability to build rapport with individuals and their families.
- A commitment to promoting independence, choice, and control for individuals with learning disabilities.
- A valid driving licence and access to a vehicle.
- Strong IT skills.
- A positive and proactive attitude.
