About the Role
As an Associate Case Manager at Ignis Care, you will play a pivotal role in supporting individuals who have sustained traumatic injuries. Your responsibilities will include assessing client needs, developing care plans, coordinating medical treatments, and collaborating with multidisciplinary teams to ensure optimal outcomes. Key Responsibilities
- Managing complex cases of individuals with Brain & Spinal Injuries.
- Coordinating with healthcare providers, therapists, and other specialists.
- Developing tailored rehabilitation and care plans.
- Providing emotional support and guidance to clients and their families.
- Monitoring and adjusting care plans as needed.
- Ensuring timely delivery of care services and treatments.
What We Are Looking For
- Experience in managing traumatic injuries, with a focus on Brain & Spinal Injuries.
- A background in Case Management within health, rehabilitation, or social care.
- Strong organizational skills and the ability to work autonomously.
- Excellent communication and interpersonal abilities.
- A compassionate, client-focused approach.
- Relevant qualifications in healthcare or case management.
