Under general supervision, actively incorporates our “Philosophy for Person-Directed Care.” Develops awareness of residents’ interests and needs, and assists residents to maximize their rehabilitation, independence and participation. Supervises nursing team and provides care and assistance of residents in the assigned care area; effectively plans and implements nursing services to provide quality care.

**  $26.75- 31.25/hr.  **  Shifts: On-Call 3PM-8PM - Days will vary, Weekends   

To be successful in the role, you would have:

  • Current LVN/LPN license in the state for which applying
  • Current CPR certification
  • Prior nursing experience: senior care, skilled nurse, post-acute or sub-acute care preferred

Work Duties

  • Leads caregiving staff, including instructing, assigning and reviewing work, planning, maintaining standards, observing and reporting on employee issues and recommending discipline;
  • Takes responsibility to lead the team and ensures that all procedures are followed during shift;
  • Maintain communication with doctors, laboratory, pharmacy and residents’ families concerning resident care;
  • Responds to emergency calls in residential living and assisted living
  • Conducts initial annual resident assessments as well as assessments for change of condition
  • Performs health updates, interviews, and charting; maintains daily records, takes doctors’ orders
