Summary:
Provides, coordinates, teaches, AND supervises nursing care for a group of patients in the setting of the patients' home. Employs the nursing process to develop and implement plans of care for patients requiring in-home health and support services. In addition to specific and skilled treatments and procedures, the nurse provides health teaching to patients and care givers, preventive health screening, referral and coordination of services in the agency and the community, and advocacy for the patient in obtaining health-related goals. The Case Manager ensures adequate service provision, including scheduling of visits, based on patients identified needs and priorities for care. Complies with all Home Health Services policies and procedures.

Work Location: Baltimore County, Baltimore City, Harford County, Howard County, and/or Anne Arundel County
Work Hours: must be available from 8:30a-5p. However, patient visit times are flexible.

To succeed in this role, you will need the following:
- Associates in Nursing required; BSN preferred
- Current RN licensure in MD or willingness to obtain needed license.
- Active American Heart Association or Red Cross BLS CPR
- Valid Maryland drivers’ license and current auto insurance, working vehicle;
- At least 2 years of Acute Care, ICU or ED experience required
- 2 years of home care experience preferred
- Passion for educating patients in their home environment a MUST.
- Experience with electronic documentation (i.e. Epic).
Salary Range: Minimum 39.94 per hour - Maximum 61.91 per hour. Compensation will be commensurate with equity and experience for roles of similar scope and responsibility.
In cases where the range is displayed as a $0 amount, salary discussions will occur during candidate screening calls, before any subsequent compensation discussion is held between the candidate and any hiring authority.
