Compensation:
$104,868.00 - $130,268.00 Annual

What You Will DoÂ
Practice independently in the communityas part of an interdisciplinary care team.Â
Deliver personalized nursing and care management to patients in their home or care facilities.Â
Constantly evaluate evolving patient needs and respond with plan of care adjustments.  Â
Qualifications
Licenses and Certifications:
License and current registration to practice as a registered professional nurse in New York State required
Valid driver's license or NYS Non-Driver photo ID card, may be required as determined by operational/regional needs.
Education:
Bachelor's Degree in Nursing
Work Experience:
Minimum of one year of recent experience in maternal/child health required
Demonstrated basic computer skills required
