The NP/PA will work in a mobile practice, primarily serving a long-term care setting.
ESSENTIAL FUNCTIONS:
- Performs on-site screening clinical assessments to determine appropriate patient treatment plans of residents of client facilities by the completion of a comprehensive assessment form
- Functions independently to perform appropriate history and physical for complex acute, critical, and chronically ill perioperative patients
- Orders and reviews diagnostic and therapeutic tests relative to patient's age-specific needs
- Assists with the provision of care in accordance with facility, state, and federal regulations
- Monitors the effectiveness of interventions
- Maintains relationships with all appropriate contacts in facilities
- Educates family members, clients, and prospective accounts regarding services and programs offered by Primary Vascular Care
- Assists with the development and maintenance of physician relations, as assigned
- Completes weekly, monthly, and other reports, as assigned, in a timely and comprehensive manner
- Bachelor's Degree required
- Degree from an accredited Nurse Practitioner or Physician Assistant Program required
- Current NP License required
- BLS - Basic Life Support through American Heart Association required
- ACLS - Advanced Cardiac Life Support through American Heart Association preferred
- Current Infection Control Certification required
- Board Certified/Board Eligible required
- Physical, Vaccines and Titers: PPD/TB, Hep B, MMR, Varicella required
- COVID and Flu Vaccine preferred
- 1 year of NP experience preferred
