Job Summary:

The Behavioral Health Consultant (BHC) provides brief, evidence-based, solution-focused assessments and interventions geared toward functional restoration, rather than symptom elimination. This is not a colocation of behavioral health services, but a behavioral health model that is interconnected with patients medical care and medical care team. The BHC functions effectively in a fast pace, high volume setting. The BHC is someone who is adaptable, flexible, and able to function independently. As part of an integrated care team, provide consultation to medical and allied health providers and contribute to a clear treatment plan for patient management.


Essential Responsibilities:


  • Possess solid working knowledge of biopsychosocial models of human behavior and development.

  • Ability to apply first line interventions in a time limited setting for a broad spectrum of physical and mental health problems experienced by patients and families throughout the lifespan as presenting in Primary Care (Pediatrics, Internal Medicine, and Family Medicine)

  • Targets of treatment may include Mental Health concerns, Substance Abuse concerns, Adverse health Behaviors (including their contribution to chronic medical illness), and Stress related Physical symptoms, ineffective patterns of health care utilization, and adjustment to new and chronic medical conditions.

  • Provide patient and family education effectively and sensitivity to encourage compliance with medical and behavioral treatment plans

  • Ability and willingness to teach classes and groups related to the integrated behavioral health provider role (e.g., chronic pain management, diabetes and stress, insomnia, ADHD, etc.)

  • Effectively utilize electronic health record and verbal communication to concisely communicate relevant information to medical providers

  • High level of comfort in providing consultation and training to other members of the primary care team in the areas of mental health, chemical dependency, and behavioral medicine.

  • Possess skill and expertise to discuss common psychotropic medications with a patient, including indications, effects and side effects, and demonstrate the ability to appropriately recommend initiating medication to primary care providers.

  • Possess knowledge about adult and pediatric screening instruments

  • Demonstrate the ability to identify substance abuse problems of patients presenting with medical complaints

  • Demonstrate ability to assess for and manage suicidal and homicidal risk, as well as concerns for child or elder abuse and neglect, using effective and evidenced-based methods (PHQ9, Columbia Suicide Risk Assessment).

  • When applicable, coordinate with the crisis team, contract police and/or Child Protective Services, write M-1 holds, and communicate effectively with receiving emergency department.

  • Perform clinical, compliance, and administrative duties related to Medicare billing

  • Other duties as assigned


Basic Qualifications:

Experience


  • Minimum two (2) years of post masters practice experience under appropriate supervision in applied Psychotherapy or providing integrated behavioral health in a primary care setting with Colorado licensure as a Licensed Clinical Social Worker (LCSW), Licensed Professional Counselor (LPC), Licensed Marriage and Family Therapist (LMFT).


Education


  • Masters degree in social work or related behavioral health field.

  • Specific primary care behavioral health/collaborative care training as part of Masters degree training program or recent integrated care certificate program completion.


License, Certification, Registration


  • Professional Counselor License (Colorado) required at hire OR Licensed Social Worker (Colorado) required at hire OR Licensed Marriage and Family Therapist (Colorado) required at hire



Additional Requirements:


  • National Provider Identifier (NPI) and Taxonomy code required at time of hire.

  • Ability to function effectively in a fast-paced Primary Care setting requiring ability to manage multiple competing demands, constantly re-prioritize, and succinctly communicate relevant information effectively

  • Ability to function effectively in an inter-disciplinary team

  • Knowledge of Psychiatry and Addiction Medicine patient populations in inpatient and outpatient settings

  • Knowledge of the complex systems of psychiatric care and substance abuse treatment at the community and county levels

  • Outstanding communication and organizational skills.

  • Consistently demonstrates the knowledge, skills, abilities, and behaviors necessary to provide superior and culturally sensitive service to colleagues, members, customers, contracted providers, and vendors.

  • Working knowledge collaborating with team members and family members, accessing internal and external resources.

  • Working knowledge of data collection tools, databases and word processing programs.

  • Demonstrated customer service skills, customer focus abilities and the ability to understand Kaiser Permanente customer needs.
