Job Summary
Will serve as a member of a multidisciplinary Palliative Care team with liaison responsibilities with hospitals and other community organizations. This position includes facilitating family meetings, working with anticipatory grief and difficult family dynamics, understanding placement options, and appropriate social, financial, and legal services.Job Duties
- Provides outpatient psycho-social evaluation for advanced illness patient population.
- Develops plan of intervention, caseload can consist of inpatients and outpatient as well as family members.Â
- Documents in the electronic health record
- Communicates effectively on an interdisciplinary level throughout the continuum of care
- Participates actively as a team member in meetings and demonstrates a professional working relationship with all levels of staff
- Regularly acts as a liaison between medical and nursing staff, patients and outside agencies.
- Attends to the needs of the advanced complex illness population
- Participates in in-service and staff development activities by attendance, serving on planning committees and or offering topics for pertinent educational programs
- Accepts personal responsibility for professional growth and development and own work responsibilities
- Pursues knowledge of palliative care and its benefit to patients and families; attends conferences and seeks out learning; pursues advanced certification in palliative care social work.
- Communicates appropriately using good interpersonal skills
- Serves, manages and supports internal and external customers
- Participates in performance improvement activities
- Accurately code services using current billing codes as applicable with system/department procedures
