The Social Services Designee/Discharge Planner is responsible for assisting and coordinating with the facility's Social Service Department to streamline the process for resident/guest discharge planning.

Responsibilities:

  • Meeting the resident/guest on or shortly after the admission and following them until they discharge to the community.
  • Attend the 72 hour care plan meeting.
  • Confirm contact information on face sheet.
  • Ordering or recommending resources to acquire durable medical equipment as needed/recommended by therapy for mobility or nursing to attain optimal health status and ordered by practitioner.
  • Arranging home health care for after discharge with the resident/guest preferred provider, as possible.
  • Ordering and coordinating  IV home infusion services, wound vacs, trach supplies, tube feeding supplies, vents and vent supplies, if necessary.
  • Coordinating nursing education with proposed caregiver.
  • Overall coordination of the discharge.
  • Attending care conferences.
  • Attending Medicare Meetings.
  • Any item related to the discharge process for residents who will be going home.
  • Ensuring completion  of the Post Discharge Plan of Care and Summary.

Education and/or Experience:

  • High school diploma/GED.
