Royal Oaks in Bradbury, CA, one of several HumanGood communities awarded Best CCRC-2024 by U.S. News & World Report, is seeking a Part-time, Temporary MDS Nurse LVN. The MDS Nurse LVN coordinates MDS schedule under supervision and direction of an MDS Coordinator-RN. S/he completes MDS sections per RAI manual and assures the accuracy of the MDS process and coordinates to ensure MDS is completed and signed by an RN Assessment Coordinator.

To be successful in the role, you would have:

  • Completed an Accredited Licensed Nursing Program (LVN/LPN)
  • Minimum three years’ experience as an LVN/LPN.
  • Minimum one-year experience as an MDS Nurse LVN/LPN preferred

A day in the life may include:

  • Coordinates MDS schedule under supervision and direction of RN MDS Coordinator.
  • Completes MDS sections per RAI manual and assures the accuracy of the MDS process and coordinates to ensure MDS is completed and signed by an RN Assessment Coordinator.
  • Transmits completed MDS to CMS and reviews final validation reports to track errors and review warning messages to correct.
  • Reviews pertinent hospital records to determine the most appropriate MDS ARD, projected HIPPS codes and identify the most appropriate principal diagnosis for skilled coverage.
  • Validates HIPPS codes and RUG classifications and ICD-10 coding through timely and accurate triple check meetings.
  • Attends PPS meetings and ensures skilled residents have the appropriate skilled coverage, length of stay and receive the appropriate care. Ensures technical and clinical eligibility are met for skilled residents. Verifies billing aspects of Medicare are thoroughly documented and reimbursed as appropriate.
  • Maintains up-to-date knowledge of CMS and State regulations, including bulletin updates.
  • Performs quarterly and notable change reviews on each resident and updates care plans as appropriate.
  • Regularly reviews CMS reports, analyze data and share with leadership team for quality improvement.
  • Works with RN MDS Coordinator and/or DON in managing survey-related reports such as CMS-672 Resident Census and Conditions and CMS-802 Matrix for Providers.
  • Participates in HumanGood provided Medicare and MDS webinars and education in order to keep current with RAI updates and evidence-based practices in the Post-Acute Long-Term Care settings.
  • Assists with Charge Nurse duties when needed.
  • Assists Licensed Nurses and other IDT members as needed.
  • Performs other related essential duties as required.
