Under minimal supervision, this role provides leadership and oversight while serving as a resource for all HumanGood Health Centers (Skilled Nursing Facilities) specific to RAI process, the Medicare program, and Medicaid Case Mix as applicable. In close collaboration with communities and the leadership team, the position drives timely and accurate resident assessments by working alongside community MDS Coordinators and the interdisciplinary team (IDT) to ensure full compliance with federal, and state regulations, while aligning with HumanGood’s standards, policies, and processes.
- Mentors and assists new MDS Coordinators in conducting resident assessments, developing plans of care, evaluating residents’ responses to interventions and documenting clinical information essential for the completion and accuracy of the MDS.
- Educates new MDS Coordinators on the RAI manual and all applicable timelines for resident assessments and completion of Minimum Data Sets. Facilitates training to enhance their skills set and knowledge through in-person visits and remote meetings.
- Collaborates with MDS Coordinators, Directors of Clinical Services, and facility leadership to enhance skill development, improve work quality, and support employee productivity and retention.
- May be required to take on the responsibilities of interim MDS Coordinator for a Community, as necessary.
- Provides MDS education including best practices to members of IDT (i.e. DON, MDS Coordinator, Social Services, Activity, Dietary, Medical Records, Therapy, and Nursing) to ensure consistency and compliance with CMS regulations and alignment with the organization’s objectives.
- Oversees Health Center RAI and PDPM process. Ensures accurate and timely completion of resident assessments, in accordance with Medicare, Medicaid, OBRA and other payer program requirements.
- Ensures Resident Assessment Instrument (RAI) guidelines are followed with focus on resident care and accurate financial reimbursement through the MDS process.
- Conducts regular audits to ensure accuracy of MDS and PDPM process. Provides education on findings to promote the implementation of recommendations and compliance with corrective actions.
- Provides oversight and education of effective IDT meeting, Medicare/Utilization meetings, and Triple Check meetings through in-person visits.
- Maintains up-to-date knowledge of CMS and state regulations, including bulletin updates.
- Remains current on legislation and regulations that may impact community operations; interfaces with government regulatory offices as a management agent and ensures regulatory compliance; maintains effective working relationships with representatives of State and local agencies, industry peers, and others involved in government.
- Supports implementation of the compliance and ethics program and coordinates with applicable consultants and community clinical leadership to ensure community plans of actions, are completed timely and work systems, processes, and outcomes meet applicable Federal and State standards.

To be successful in this role, you would have:Â
- Bachelor of Science in Nursing
- Current CA RN license, licensed to states where HumanGood operates preferred
- Current CPR/BLS certification
- MDS RAC-CT Certification
- Complete RAC-CTA within 6 months of hire
- 5+ years working as RN MDS Coordinator
- Thorough knowledge of RAI, Medicare and Medicaid regulations
- Proficient computer skills necessary to complete documentation, review electronic health records and communicate with team members

Compensation: $110,000-130,000 depending on experience and location + outcome/performance base incentive
Location: This is a remote position with occasional travel (up to 15%). Location in Southern California is strongly preferred.Â
