Physical Therapist - Home Health - (PT - HH)

How long is the contract assignment? 13 weeks 2) Schedule of assignment hours? M – F, 8am – 5pm 3) Will this candidate be visiting patients in care facilities or only in patient’s homes? Both 4) Will you want to hire the contractor at the end of the contract or is this truly a contract assignment? Contingent on performance6) What region are you in? Home health 7) What’s creating the request? Only one FT PT in CC 8) To which location will this person report for assignment, full address, Including the OU (Location Code) 3505 Duluth Park Lane Suite 300 Duluth GA 30096 (8315) Please confirm above location is correct and list any other territories that are to be covered by the candidates: Norcross, Peachtree Corners, Duluth. Candidate must be willing to cover other areas as needed. 9) How many miles per week will the consultant travel? Varies according to patient needs 10) What are the weekly units of productivity expectations? 30 11) What EMR systems will the consultant be using: HomeCare HomeBase, Oasis, Outlook, teams, etc. 12) Will there be any on call?
