Acute Hospital Care at Home (AHCaH) Advanced Practitioners (APs) operate similarly to inpatient hospitalists with a more virtual care approach. Cooperating with other team members through huddles and as needed to assess patient needs and identify appropriate patients for this model of care at our off-campus office in Pittston, PA.
