Cooperating with other team members through huddles and as needed to assess patient needs and identify appropriate patients for this model of care at our off-campus office in Pittston, PA. As the core provider on the AHCaH team, you’ll work closely with patients and collaborate with a dynamic interdisciplinary care team under physician guidance. Key responsibilities include conducting in-person and remote patient assessments, developing and implementing treatment plans, and providing ongoing monitoring and coordination with field staff.
