This position is full time (40 hours/week) Monday - Friday. Employees are required to have flexibility to work any of our 8-hour shift schedules during our normal business hours of 8:00 am – 5:00 pm. It may be necessary, given the business need, to work occasional overtime. Our office is located at 1580 VALLEY RIVER DR, Eugene, OR.

We offer 4-8 weeks of on-the-job training. The hours of the training will be aligned with your schedule. 

Primary Responsibilities:

  • Perform assessments to determine best teaching methods and barriers to care that might affect patient’s ability to comply with treatment.
  • Provide education and information to patients and their families, paying particular attention to providing education in a manner most suitable for an effective patient learning experience.
  • Provide ongoing follow-up, basic motivational interviewing and goal setting with patients/families.
  • Provide education that assists patients in identifying goals for behavior change that are necessary for healthy
  • lifestyle changes. Assist with recording of mutually agreed upon goals.
  • Provide referrals for services to community agencies as appropriate. Help patients connect with transportation
  • resources and give appointment reminders in special circumstances.
  • Follow-up with patients via phone calls or at clinic visits, and document accurately in EMR system.
  • Identify high ED utilizers and assist in creation of a care plan to help the patient obtain care through their PCP’s
    office.
  • Identify patients discharged to SNFs and tracking patients to identify when they are discharged to home.
  • Assist patients in locating housing and food opportunities.
  • Collaborate with Case Management in assisting patients locate appropriate Behavioral Health resources.
  • Assist patients with accessing insurance and welfare programs.
  • Work with pediatric population in assisting parents with identifying special programs for their child.
  • Screen patients for social determinants of health.
  • Other duties as assigned

Required Qualifications:

  • HS Diploma/GED
  • 18 years or older
  • Certification in Community Health Work required.
  • Healthcare provider level CPR/BLS certification required
  • Ability to work any of our 8-hour shift schedules during our normal business hours of 8:00 am – 5:00 pm, Monday to Friday. It may be necessary, given the business need, to work occasional overtime. Our office is located at 1580 VALLEY RIVER DR, Eugene, OR.

 

Preferred Qualifications:

  • 1+ years experience working in a medical field
  • Knowledge of medical terminology
  • Experience with an EMR
