The Referral and Admission Coordinator is responsible for managing the referral process for assigned agencies. The Referral and Admission Coordinator works closely with other members of the team to ensure adequate coordination and transition for patients with an order for home health services. The Referral and Admission Coordinator is responsible for processing all incoming referrals through e-portals and other methods and may have additional duties assigned. This role is primarily a remote position.


  • Responsible for the referral process through e-portals and other referral sources for assigned home health agencies.
  • Effectively communicates patient/referral information to appropriate staff members, referral sources and location/s. 
  • Obtains demographic and clinical information from clients, physician’s offices, and all other referral sources to obtain a complete referral. 
  • Delivers a seamless referring partner, patient, and agency experience by developing and implementing a service-minded culture with consistency in daily operations.
  • Provides general information about agency services to patients, families and other referral partnersas needed.
  • Effectively triages incoming calls and messages from referral sources and other partners.
  • Communicates directly with all involved including but not limited to the attending physician, referring physician, nurse, facility, patient, and families.
  • Serves as a liaison between clinical staff, referral partner and business development staff. 
  • Attends and participates in annual in-service training programs.
  • Maintains compliance by documenting accurately and timely with the organizations electronic medical record system.
  • Assists in data entry and tracking of non-admitted patients.
  • Serves a patient advocate by ensuring optimal patient outcomes.
  • Complies with all state and federal referral/intake regulatory requirements.
  • All other duties as assigned.
  • If a licensed LPN, LVN, or RN, additional clinical duties may be required of the role including but not limited to accepting verbal orders for evaluation of patient for services.

Formal Education: High School diploma required; Bachelor’s degree preferred. May be a graduate of an accredited LPN, LVN or RN school. 

License, Registration, and / or Certification Requirement: May be an LPN, LVN, or RN licensed in the state in which LHC Group operates

  • Sales support or customer service experience required.
  •  Minimum of one (1) year of healthcare experience required.
  • Strong communication skills and the ability to work both with a team and independently.
  • Computer experience including knowledge of Microsoft Office required.
  • Must have excellent critical thinking and problem-solving skills.
