Almost Family is seeking a Case Coordinator in the Fond Du Lac area. The Home and Community-Based Services Case Coordinator is responsible for overseeing the services provided to a caseload of clients on a daily basis. This responsibility includes interfacing with clients/families/employees to ensure quality service delivery. The Case Coordinator is responsible for day-to-day staffing of cases, problem solving, assisting in the hiring process, providing orientation to new caregivers, payroll/billing, and ensuring customer satisfaction. 

 


  • Coordinates the delivery of care and services to agency clients including responding to inquiry calls from clients, families, physicians, referral sources, and care managers, assigning and scheduling appropriate caregivers, and providing in-home visits as needed.
  • Participates in the orientation of agency staff and caregivers.
  • Assists with the management of the electronic visit verification process.
  • Processes the payroll and billing of client services weekly.
  • Assists in the maintenance of client and employee records including data entry as assigned.
  • Participates in the organization’s performance improvement and compliance programs.
  • Participates in the after hours on call rotation as assigned.
  • All other duties assigned.

High school education or equivalent required.

 

Skill Requirements:

  • Scheduling and data entry experience preferred.
  • Ability to coordinate many activities at one time.
  • Strong customer service skills.
