Job Description: Post-Acute Long-Term Care Nurse Navigator

The Staff Pad has partnered with a client in Montana who has a vision to be the gold standard for Healthcare. We are searching for a Post-Acute Long-Term Care Nurse Navigator to support the organizations mission. This entrepreneurial and inspiring leader will play a crucial role in coordinating patient care within skilled nursing facilities (SNFs) while ensuring the highest quality of service for patients and their families.


Responsibilities:
  • Advocate for patients and facilitate their journey through the healthcare continuum, improving satisfaction and outcomes.
  • Travel to local SNF facilities regularly to ensure continuity of care and build strong relationships with care teams.
  • Collaborate with multidisciplinary teams to create and implement individualized care plans.
  • Coordinate outpatient and virtual visits, ensuring timely scheduling of appointments and diagnostic procedures.
  • Maintain accurate documentation in patient EMRs and manage communication between SNF staff and providers.
  • Educate and support SNF staff on standard operations and care-related topics.
  • Identify improvement opportunities within care processes and implement strategies to enhance patient experiences.


  • Requirements

    Qualifications:

  • Minimum of 3-5 years of nursing experience, with at least one year in acute care preferred.
  • Graduate of an accredited nursing school; BSN preferred.
  • Current Montana RN license and BLS certification required.
  • Excellent communication, time management, and collaboration skills.
