When you join us as an Inpatient RN Case Manager, you'll be making a difference in peoples' lives and will be responsible for discharge planning and improved transitions of care of hospitalized health plan members. You will ensure patients receive quality medical care in the most appropriate setting.Â
Performs the following case management skills on a daily basis:Â
- Perform patient assessment of all major domains using evidence based criteria (physical, functional, financial and psychosocial)Â
- Develop individualized discharge plans that involve provider, patient, and caregiver goals for successful transitions of careÂ
- Implement discharge plan involving health care resources across the continuumÂ
**Candidates must be flexible to work onsite at a hospital in the Las Vegas and Henderson area.**Â
Primary Responsibilities:
- Assess and create a discharge plan to ensure patient receives quality medical care. Implement care strategies that are individualized for patients and directed toward the most appropriate and the least restrictive level of care
- Utilize both company and community-based resources to establish a safe and effective case management plan for hospitalized members and effective discharge planning to prevent readmissions
- Collaborate with patient, family, and health care providers to develop an individualized plan of care that encompasses both acute care episode and post hospital discharge plan
- Communicate with all stakeholders the required health related information to ensure quality coordinated care and services are provided timely to all hospitalized members
- Advocate for patients and families as needed to ensure the patient’s needs and choices are represented and supported by the health care team
- Work performed and completed onsite at various acute and alternative level of care facilities within the area
- Floating may be required covering cases in another facility telephonically
- Depending on EMR access/availability may be required to cross covering in multiple facilities based on the daily average census and PTO coverage
- Attend all Medical Management clinical rounds

Required Qualifications:
- Registered Nurse with active, unrestricted license in the State of NevadaÂ
- CCM certification or ability to obtain within 2 years of employment
- 2+ years of adult clinical, managed care, or case management experience
- Intermediate level of proficiency using a PC in a Windows environment, including MS Word and Excel
Preferred Qualifications:
- 2+ years of ER, ICU, IMC/stepdown or med-surg experienceÂ
- Case management/utilization review experience
- Experience in a managed care organizationÂ
- Knowledge of InterqualÂ
- Understand spectrum of healthcare delivery system including post hospital care such as SNF/LTAC/AIR criteria
The hourly range for this role is $28.61 to $56.06 per hour based on full-time employment. Pay is based on several factors including but not limited to local labor markets, education, work experience, certifications, etc. UnitedHealth Group complies with all minimum wage laws as applicable.
