As a Home Health Wound and Ostomy Registered Nurse, you will function independently and will be responsible for providing patient care in the home setting.
You will work autonomously with a collaborative and supportive team. This is a multidisciplinary team of medical professionals with various skills, experience, education and specialties all working together through the care management process. This is a team that is there for each other and communicates a lot via phone. They support each other when questions arise and help each other in making sure we provide the best care possible to our patients.
Shift: PRN-Monday-Friday daytime hours
The Work:
- Participating with interdisciplinary team, providing clinical guidance to team for complex wounds and ostomies.
- Provide consultation and direct patient care utilizing best practice.
- Evaluate wound and ostomy products, collaborating with team for product choice.
- Participating in QAPI activities including participation in annual survey preparation and survey process, and maintenance of CMS standards.
- Adhering to all nursing policies and standards, and proactively strives to maintain and improve quality outcomes.
- Must be a licensed Registered Nurse in Missouri & Kansas.
- Wound Care certification is required.

Accountable for utilizing the nursing process. Responsible for coordinating the care for a group of patients and consulting with other members of the health care team when indicated. Provides nursing care in compliance with the Nurse Practice Act.
