The Opportunity:
Saint Luke's Home Care provides care to 22 counties in and around the Kansas City Metro and has one of the lowest hospital readmission rates in the region, a testament to our high-quality care.
Saint Luke's Home Health is looking for a Home Health Aide to join their term. In this position, you will assist with ADLs and provide care to patients in their home according to the care plan provided by the care team of Nurses, Case Managers and Therapists. You will see 5 patients a day.
Shift: Part-time: Monday - Friday, No nights, No Weekends, No Holidays. 12 hours per week (8-10 visits), flexible scheduling.
The Work:
- The Nurse Aide provides care according to the patient needs as directed by the clinician.
- The Nurse Aide assists in the patient care anticipating patient family needs in a proactive fashion.
- The Nurse Aide collaborates with other health care team members to enhance the provision of patient care.
- Must obtain Home Health Aide certification within 90 days of date of hire
- CNA-Kansas and Missouri
