Job Description

Remote In-Home Services Case Manger -Pediatrics

Saint Luke's Crittenton Children's Center in South Kansas City, MO is seeking an In-Home Services Case Managerto join their Pediatric Behavior Health Team. This is a remote position that will be asked to go on site for team meetings and will work with patients in their home.

This position will be responsible for managing a case load of clients ranging in age from 4 to 18 with our contracting partner Beacon Mental Health. Duties may include treatment planning, linking with community resources, coordinating care with treatment team, and assisting families with positive coping skills. Areas served include Clay, Ray, and Platte counties.

Provide services of clinical case management to the child and family in the home and community setting.

Assist with implementing a treatment plan developed by a Master level clinician.

Develop a crisis plan.

Develop strategies for the child to succeed in the school, home and community.

Teach behavioral interventions to the parent/guardian, school, etc.

Connect the child/family to community resources.

Use "wrap-around" philosophy to provide resources for the family to include respite services.

Provide crisis support by phone after hours Monday through Friday and weekends as available.

The Work:
  • Must be independent worker with willingness to be flexible with work hours based upon the needs of the families they serve. Utilization of evidence based programming including Attachment, Self Regulation, and Competency (ARC.)
  • Research of area community resources and connecting families based upon their need.
  • Share written progress notes about progress of visits, concerns, and any noted changes with therapist and supervisor on families assigned.
  • This role will provide care to various counties around KC metro

Must have aBachelor's in Social Work, Counselingor related field anda clean driving record with applicable vehicle insurance.
