Under minimal supervision, the Social Services Designee assesses resident social service needs and ensures the identified needs are addressed. Assesses resident’s social needs, cognitive abilities, and provides support for the emotional well-being of residents and families throughout the Health Center. Advocates for the resident and family, while maintaining a professional and positive customer service approach. As a representative and team member of the company, this position is expected to present oneself in a manner that reflects professionalism and ensures resident satisfaction.
Schedule: Monday-Friday
Pay Range: $26.32 - $32.24
To be successful in the role, you would have:
- Minimum of two years of experience working with elderly in long term care facility
- Bachelor’s Degree in Social Work preferred

Work Duties

- Presents and maintains a positive customer service focus when interacting with residents, vendors, visitors and team members;
- In collaboration with the IDT, assesses residents’ social needs, cognitive abilities, and provides support for the emotional well-being of residents and families throughout the Health Center.
- Utilize consultation data to plan and coordinate resident care and rehabilitation, following through to ensure service efficacy, modifying plans to comply with changes in condition;
- Ensures that a social service assessment is completed with accurate documentation for all new residents within 5 days in the Health Center;
- Begins discharge planning upon admission for Health Center residents and follows-up with discharge planners for residents who have been admitted to the hospital;
- Assists in coordinating with the Director of Resident Services and the DON the transfer of residents to appropriate levels of care on campus;
- Participates in the weekly Resident Review process and makes giving information recommendations based on assessments;
- Ensures that social service is available and provided to residents in the Health Center;
- Handles complaints and settling disputes, resolves grievances in coordination with the IDT and DON. Refers residents and family to community resources when needed;
- Coordinates all resident care plan meetings with IDT team and MDS coordinator;
- Responsible for all related letters to family and residents in coordination with IDT team care conferences;
- Prepares progress notes on residents as needed;
- Other duties as needed;
