The Post-Acute Liaison is responsible for generating business from referrals sources to meet the community’s revenue and census goals.  The majority of this position is spent in outside sales; developing relationships with community hospitals, physician offices, community health clinics and other community outreach programs. This position contributes to the success of Human Good by coordinating external professional relationships for the community as a whole with an emphasis in skilled nursing components of senior living in an effort to achieve the highest possible occupancy within budgetary and regulatory guidelines. As a representative and team member of the company, this position is expected to present oneself in a manner that reflects professionalism and ensures resident satisfaction.

Coordination with admission coordinator and other key team members to ensure referrals and care transitions are managed efficiently to support hospital throughput needs with safely coordinated care transitions. Engagement with collaborative network leadership, risk holding physicians and managed care partners - sharing in this work with HC leadership and providing IDT insights of program enhancement opportunities to support evolving post-acute patient population needs.

Compensation: 61,680.73 to 92,521.10 + performance-based bonus
