LSF is seeking a talented Case Manager who wants to make an impact in the lives of others.

Purpose and Impact:

The purpose of the Case Manager is to conduct intakes and assessment of refugee/entrant client needs; demographic and copies of eligibility documentation information, including the Family Integration and Self Sufficiency plan LSF will determine if the client (and/or any family member) is in need of case management and wrap-around services designed to address barriers to integration and self-sufficiency. The assessment will identify risk factors for those individuals who are experiencing adjustment difficulties such as failure to gain or maintain employment, medical or mental illness, or other indicators of adjustment difficulties. 

Essential Functions: 

  • Case Manager opens a case file for the family unit, conduct an assessment, and develop a Family Integration and Self-Sufficiency Plan within the time frame required eligibility determination and intake for programs. 
  • Responsible to complete intakes and assessments on clients according to established programmatic and COA standards. 
  • Assess to determine the barriers to self-sufficiency and client’s 
  • Assess to determine case management needs such as employment, personal finances, food, clothing, housing, and health and mental health. 
  • Develops and implements family self-sufficiency plan with the client to address identified needs within established time frame. 
  • Monitors and evaluates client progress and adjust service plan and/or delivery as necessary. 
  • Empowers clients towards cultural acquisition and comprehension of the new community through an understanding of self, personal situation, and client’s responsibility for adjusting to new life in local community. 
  • Secures supplementary information in the case file related to client barrier’s removal. social factors contributing to client’s situation. 
  • Completes all necessary documentation to ensure compliance with funding requirements, licensing requirements, COA standards, and agency quality improvement standards. 
  • Coordinate with other professional’s referrals for clients. 
  • Provides transportation for clients on an as-needed basis. 
  • Conducts follow ups as required by program guidelines. 
  • Conducts home visits when needed. 
  • Applies a depth of knowledge within area of responsibility to implement assigned activities. 
  • Accepts accountability for assigned activities and adapt work style to perform independently or within a team. 
  • Advocates for clients with service providers or agencies and in the community. 
  • Participation in case reviews, case staffing, and clinical supervision.    
  • Must possess a clean/valid Florida Driver License, reliable transportation and proof of Insurance (A transcript of driving record will be requested from the Department of Motor Vehicle at the time of hire).

Physical Requirements: 

Able to work in a fast-paced environment. Must be able to work flexible and long hours.  

Education: 

BA or BS in Social Services, Psychology, Sociology, or Human Services.    

Experience: 

Must have two years of experience in case work management, counseling or related human services activities. 

Skills: 

  • Must be a self-starter, whom can work independently but able to seek out supervision and help when needed.  
  • Requires traveling within the county area as specified in the contract. 
  • Interpersonal, organizational, and problem solving, written and verbal communication. 
  • Good time management skills.   
  • Good computer skills.   
  • Must be able to work with a culturally diverse population.   
  • Must be bilingual. Or learn to effectively work with telephonic and live interpreter in meeting clients’ needs with linguistic supports. 
  • Knowledge of social services agencies and programs available in the community, client eligibility. 
  • Effective interviewing and counseling techniques. 
  • Must be able to successfully balance multiple activities, in support of program or departmental goals. Manage contacts with internal and external stakeholders to ensure flow of information and coordination of efforts. 

Other: Must demonstrate sensitivity to our service population’s cultural and socioeconomic characteristic and needs.  

Principal Accountabilities: 

  • Reports to the Program Manager.  
  • Adheres to LSF policies and procedures. 
  • Must be a team player with co-workers and social service agencies. 
  • Provides concise, complete and accurate
