Job Summary

Leads the Clinical Practice Specialist team and Patient Navigation initiatives. Manages the patient navigation program's operations, growth, and strategic direction. Leads patient navigators to serve as the subject matter expert in driving program processes and outcomes to continuously improve timely access and high quality of care for patients and families. Leads the clinical practice specialist team in implementing strategies to promote knowledge of evidence-based practice, research, and quality improvement methodologies to identify opportunities for process/outcome improvement and implement sustainable practice changes.  Responsible for the development, oversight, and management of program processes performed by patient navigators for patients with special health care needs within each of their specialties. Support in evaluating and identifying disparities/social determinants of health, special health care needs/conditions, and/or other factors impacting patient ability to access and coordinate needed care. Responsible for providing nursing excellence leadership, applying strategies, and overseeing evidence-based practice. 

Job Specific Duties

  • Provide direct leadership and oversight of the development and implementation of a framework for Patient Navigation strategy and growth including 1) working in collaboration with the Nursing Professional Practice Director, VP/CNO, and specialty areas leadership to ensure Patient Navigation quantitative and qualitative program data collection; 2) Reporting on relevant data (quantitative and qualitative) related to the Patient Navigation program to the relevant stakeholders; and 3) promoting staff education and organizational awareness. 
  • Support development measurable program goals and outcomes for patient navigation. Develops updates to impacted policy, process, and procedure and lead the development of program processes specific to outreach and integration with primary care medical provider, medical specialists, and medical health plan. 
  • Oversight of development of comprehensive professional care plans, based on provider treatment plan, evidence based chronic care guidelines, and patient/family goals for patients with chronic conditions and/or recent care transitions.  Promote treatment acceptance and adherence to provider recommendations and instructions.
  • Oversight and clinical direction for the enrollment, assessment, and facilitation of management services for patients with special health care needs through oversight and management of the patient navigator program processes. 
  • Coordinates and collaborates with stakeholders such as interprofessional teams, internal, and external customers to create collaborative work relationships that drive performance and quality outcomes in a timely and effective manner. 
  • Develops and maintains detailed project work plans, budgets, and communicates status to key internal clients and stakeholders throughout the organization. 
  • Provides direct leadership and oversight of the Clinical Practice Specialists, Nurse Navigators, and Patient Navigators. 
  • Maintains current understanding of DNV and other accrediting body standards and ensures adherence to these standards.
  • Manages procedures including payroll, time/attendance, and progressive disciplinary action in a timely and accurate manner. 
  • Accountable to oversee and evaluate staff competencies, regulatory requirements, credentials, accuracy and completeness of paperwork.
  • Supervises team members and management the performance of individuals through ongoing coaching, feedback and development to motivate, engage and drive a high performing team.  Collaborates with TM&E to develop recruitment plans based on labor needs, participates in internal and external job candidates selection including screening, conducting interviews, and onboarding. 
  • Supports development of manuscripts for publication and presents at nursing professional conferences. 
  • Disseminates pertinent safety and specialty related information and contributes to the development and implementation of best practice guidelines. 
  • Responsible for effective supervision and application of Quality and Process Improvement principles and methodologies.

Qualifications

Minimum Job Requirements

  • Master's Degree - graduate of a state approved school of nursing 
  • Registered Nurse Licensure within the State of Florida or Multi-State Enhanced Nursing License Compact (eNLC) – maintain active and in good standing throughout employment
  • CPR - American Heart Association BLS - maintain active and in good standing throughout employment
  • 3-5 years of pediatric RN experience in an acute care healthcare environment
  • 2-4 years of leadership experience

Knowledge, Skills, and Abilities

  • National Certified Nurse Leader (NE-BC or CNML) or Nursing Professional Development Certification (NPD-BC) within 5 years of hire preferred.
  • Proficient with computers, EMR, and Microsoft Office preferred.
  • Strong customer service, problem solving, critical thinking, time management, and conflict resolution skills.
  • Ability to work through others, influence stakeholders, and staff to achieve a common goal.
  • Effective verbal and written communication skills.
  • Ability to positively influence, hold self, and others accountable.
  • Ability to effectively be the liaison with physicians and physician teams.
