We are hiring an RN Field Team Supervisor  to work with Willcare Home & Community Based Services in our Middletown, NY office! $5,000 Sign On Bonus (dividedpayout)   This is a full time position with an excellent schedule - Monday through Friday 8:30am - 5:00pm (with occasional on-call hours).  We also have an excellent benefits plan that includes medical/dental/vision, PTO, and competitive pay: $80,000-85,000/year

The Clinical Manager is responsible for the coordination of high quality, cost-effective patient care. This position is responsible for the operations and operational flow of the agency including performance of and/or oversight of crucial activities including scheduling, staffing, intake, physician communications, billing and reimbursement process, and overall compliance.

The Registered Nurse in Home and Community Based Services trains and directs provisions of nursing and personal care to patients in their homes as prescribed by the physician or as authorized by the state/program and in compliance with applicable laws, regulations, and agency policies. The Registered Nurse coordinates the plan of care with other health care professionals involved in care and helps to achieve and maintain continuity of patient care by planning and exchanging information with physician, agency personnel, patient, family, case manager and other community resources.

Specific Job Duties/Responsibilities 

  • Provides high quality clinical services within the scope of practice and within infection control standards, in accordance with the plan of care/service plan, and in coordination with other members of the patient/client’s
    care team from admit through discharge.
  • Completes clinical nursing assessments in accordance with federal and/or state program requirements and as required by payer

Experience Desired

  • a minimum one year RN work with a Certified Home Health Agency is required

License Requirements

  • Must have current RN licensure in state of practice.
  • Current drivers license, vehicle insurance and access to a dependable vehicle or public transportation.
  • Current CPR certification required.
