Job Overview:
The Occupational Therapist (OT) administers occupational therapy to patients on an intermittent basis in their place of residence. This is performed in accordance with physician orders and plan of care under the direction and supervision of the Clinical Manager.

The Occupational Therapist must be able to interface well with professionals within and outside of the organization, as well as with patients and their families. A high degree of integrity is a must. Proven ability to maintain discretion, handle confidential information, and demonstrate the highest level of ethics and compliance. Exercises good judgment in a variety of situations, documents timely and with clarity and accuracy at all times to ensure regulatory compliance and integrity of work product. The Case Manager will be a team player, is effective in the role, has a strong ethical and moral compass, and adheres to any/all of EverCare’s compliance policies.
Primary Responsibilities:
- Improves or minimizes residual physical disabilities of the patient.
- Returns the individual to optimum and productive living within the patient's capabilities.
- Periodically participates with all other health care staff in patient care planning.
- Provides prescribed occupational therapy.
- Directs and supervises staff as required.
- Takes initial history and makes initial evaluation.
- Performs all skilled procedures.
- Consults with physicians regarding change in treatment.
- Writes reports to physicians regarding patient's progress.
- Instructs patients/family members in home programs and fine motor movement exercises.
- Periodically presents an in-service to the Program staff.
- Assists the physician in evaluating the level of function.
- Completes an initial evaluation, including plan of treatment and goals, and submits to the physician for approval.
- Recertification by the physician every 60 days is necessary if further treatment is to be continued.
- Writes a progress note for each visit.
- Completes a reevaluation when the expected duration of treatment is reached.
- Writes a summary upon the patient's discharge.
- Helps develop the plan of care and revises as necessary.
- Prepares clinical and progress notes.
- Advises and consults with the family and other Program personnel.
- Participates in in-service programs.
- Maintains knowledge and skills through active participation in training, in-services, and conferences.
- Performs other duties and tasks as assigned by the supervisor to ensure optimal department functioning.

Requirements:
- Proof of current OT license in good standing from the State Of New York
- Two (2) years’ experience, preferred
- Graduate of an Occupational Therapy curriculum accredited jointly by the Committee on Allied Health Education and Accreditation of the American Medical Association and the American Occupational Therapy Association (AOTA) and/or certified by successfully completing the National Certification examination
