Job Overview:

A Registered Physical Therapist (PT) administers physical therapy to patients on an intermittent basis in their place of residence. This is performed in accordance with physician orders and plan of care under the direction and supervision of the Director of Rehabilitation.

The Physical Therapist must have very strong interpersonal skills and the ability to build relationships with stakeholders, including staff and external partners, with emotional maturity.

A high degree of integrity is a must. Proven ability to maintain discretion, handle confidential information, and demonstrate the highest level of ethics and compliance. Exercises good judgment in a variety of situations, documents timely and with clarity and accuracy at all times to ensure regulatory compliance and integrity of work product. The Physical Therapist will be a team–player, effective in the role, with a strong ethical and moral compass adhering to any/all of EverCare’s compliance policies. 

Primary Responsibilities:

  • Understands and adheres to established policies and procedures.
  • Provides physician-prescribed physical therapy.
  • Improves or minimizes residual physical disabilities of the patient.
  • Returns the individual to an optimum and productive level within the patient's capabilities.
  • Participates with all other health care personnel in patient care planning.
  • Directs and supervises personnel as required.
  • Takes initial history and makes initial evaluation.
  • Performs all skilled procedures as ordered by the physician.
  • Consults with physicians regarding changes in treatment.
  • Writes reports to physicians regarding the patient's progress.
  • Instructs patients and family/significant others in home programs and activities of daily living.
  • Participates in in-service programs and presents in-service programs as assigned.
  • Participates in performance improvement activities as assigned.
  • Attends all patient care conferences as scheduled.
  • Prepares clinical and progress notes.
  • Assists physicians with evaluating levels of function.
  • Helps develop the plan of care and revises it as necessary.
  • Consults with family and Agency personnel.
  • Completes and submits OASIS assessments, reassessments, transfers, resumptions of care, discharges, and significant changes in condition in accordance with Agency-defined timeframes.
  • Appropriately utilizes ICD-9 codes.
  • Maintains knowledge and skills through active participation in training, in-services, and conferences.
  • Performs other duties and tasks as assigned by the supervisor to ensure optimal department functioning.

Requirements:

  • Proof of current PT license in good standing from the State Of New York
  • Two (2) years’ experience, preferred
  • Graduate from a Physical Therapist curriculum approved by the American Physical Therapy Association, the Committee on Allied Health Education and Accreditation of the American Medical Association, or the Council of Medical Education of the American Medical Association and American Physical Therapy Association
  • Acceptance of the philosophy and goals outlined
  • Ability to exercise initiative and independent judgment
