Position Summary: The Care Manager is responsible for ensuring that all residents have a person-centered, Individualized Service Plan (ISP) based on each resident’s goals, capabilities, interests, and physical, spiritual, and cognitive needs. This position functions in collaboration with all members of the interdisciplinary team, including the resident, family, PCP, specialists, acute and post-acute providers, to coordinate and facilitate quality care while minimizing fragmentation of the care and service delivery system for Inspīr residents. The Care Manager is responsible for performing compressive assessments on residents at regular intervals and as needed so that the ISP is always up to date. The role includes a compendium of care management/care coordination functions geared towards the development and implementation of an ISP that promotes whole-person wellness, and avoids unnecessary utilization of EMS, ED, hospital admissions/re-admissions and SNF stays or pre-mature discharge.
Essential Functions:

- Provide care management services, psychosocial support and community linkages to Inspīr residents; In collaboration with the interdisciplinary care team and external provider partners, integrate and coordinate wellness, primary care, behavioral health, rehab, pharmacy, acute, subacute, palliative, and hospice services to residents to optimize outcomes
- Provide psychosocial support, consultation and community linkages/resources to family members of residents, including helping navigate the complexities of the healthcare system
- Perform comprehensive assessments on residents requiring advanced assessment/care planning; or who may be experiencing acute episodes, crises, exacerbation of chronic conditions, or transitions of care. Assessments may be performed in various settings including home, community, hospital or SNF
- Perform post discharge assessments from acute facilities or skilled nursing facilities to include care coordination, resident/family education, and Post D/C Action Plan designed to promote wellness and avoid readmission
- Communicate regularly with Interdisciplinary Team members, external providers and resident family members regarding resident status, changes in condition, or updates to care plan
- Participate in regular RCT meetings, external provider partner meetings, and liaise with community based providers as needed
- Act as a clinical resource and mentor to Inspīr staff and other members of the Interdisciplinary Team providing advanced clinical knowledge, skill, and expertise and consultation and support; Help to foster clinical excellence, accountability, and reliability among team members
- Ensure complete, timely and appropriate documentation of encounters and care management duties and activities in the EHR and external EHRs as indicated
- Be readily available and accessible via phone, email and text during working hours and respond to requests promptly
- Actively participate in self-performance evaluations and engage in continuing education to maintain advanced competency
- Participate in ongoing quality improvement efforts and initiatives
- Contribute to innovation efforts and participate in innovation pilots
- Manage and build professional relationships with geriatric professionals and similar professional colleagues
- Demonstrate innovative, entrepreneurial thinking in response to competitive trends and business development opportunities and respond/adapt to market/industry trends
- Consult/collaborate with Inspīr senior leadership
- Provide mentoring and shadowing experiences to new team members to aid in their orientation and onboarding
- Participate in professional growth, development and education opportunities in an effort to be relevant and current for ongoing changes/trends in geriatric care, aging, Alzheimer’s disease, related dementias, and other best practicesÂ
Education/Experience/Licensure/Certification
- Minimum of Bachelor-level NY-state licensed Registered Nurse (RN) in good standing, preferably with advanced degrees/training and certification(s)
- Minimum of 1 year of care management experience
- Minimum of 2 years' experience in hospital, senior living, LTC, SNF, and/or home health care
- Minimum of 2 years working with elder adult population with complex medical needs including dementia
- Basic first aid and CPR certification
- Proficiency with Microsoft Word and Excel and use of Electronic Medical Record
