Memory Care Coordinator
Reports To:Â Memory Care Director
Summary: The Memory Care Coordinator is responsible for assisting the Memory Care Director with meeting Maplewood Senior Living standard and practices along with state and federal regulations as it relates to memory care and programming. The memory care coordinator is responsible for the development, coordination’s, and implementation of all programs and ensuring that it is therapeutic to the residents and maximizes their quality of life.
Essential Functions:
Management Responsibilities
- In Absence of the Memory Care Director, be involved in the interviewing of any associate who will be assigned to any of the specialized memory care programs, including RSAs, housekeeping, and activity positions.
- Provide outstanding customer service to all residents, family members, responsible persons, and visitors of residents in the community.
- Assist with ensuring Memory Care Program meets all state, federal and other regulations.
- Demonstrate requisite knowledge, skills and abilities consistent with the duties & responsibilities of this position.
- Personable, reliable and a great communicator and team player.
- Able to relate professionally and positively to residents, physicians, government regulators, incoming visitors and associates at all levels.
- Provide ongoing communication to the Memory Care Director, Resident Service Director and Executive Director regarding staffing issues or concerns.
- Other responsibilities as assigned by supervisor (Memory Care Director/Executive Director).
- Attend Monthly All Associate meetings.
- Attend bi-Monthly Regional Memory Care Coordinator meetings.
Care, Service Plans and Wellness
- In the absence of the Memory Care Director, Participate in weekly tracking meetings to assure that daily information and observations
- Assist with ensuring, residents are all groomed and dressed properly and according to personal preferences (i.e. jewelry, clothing style, etc.)
- In the Absence of the Memory Care Director, Participate in weekly tracking meetings to assure that daily information and observations by MCD/MCC is utilized.
Staff Training
- In the absence of the Memory Care Director, will provide Habilitation Therapy Training to Orientation classes.
- In the Absence of the Memory Care Director, will provide the Heart Philosophy Training, Basics of Dementia and wandering and Elopement overview during new hire orientation.
- In the Absence of the Memory Care Director, is responsible for providing monthly dementia in-services to all associates that relates to dementia. Curriculum will be provided by Maplewood Senior Living.
Therapeutic Recreation Program
- Ensures a monthly program calendars is designed and implemented based on residents’ abilities, interests, hobbies, religions and work/volunteer experience.
- Plans, implements, and conducts/monitors recreational programs to meet the needs of all of our resident of our memory care population.
- Select, schedule and lead activities appropriate for all levels of memory-impaired residents.
- Assist with the coordination transportation of residents for outings and events outside of the community, which may include driving a community motor vehicle.
- Successfully work within budgetary guidelines regarding entertainment, supplies and labor hours.
- Assist with planning and assisting with special events at the community as needed.
- Assist with the completion of Staying Connected Form/Personal History Form for all residents in our memory care program.
- Assist with the coordination of WISH Program/Parallel activities with Resident Service Care team.
- Ensure and maintain appropriate program supplies for recreation programs.
- Assist with the purchasing of adaptive program supplies as needed to meet the needs of the residents.
- Assist with keeping current a list of local entertainers who will provide entertainment for the memory care residents.
Sales & Marketing
- In the Absence of the Memory Care Director, assist with touring and providing information about our memory care program to families of perspective residents.
- Program Coordinator is actively involved in a minimum of one committee or activity for the Alzheimer’s Association.
- Develop and distribute a monthly newsletter.
Dining Program
- Assist with providing supervision in the dining room and is offering necessary assistance in an adult and dignified manner.
- Assist the MCD to ensure tables are set with all needed utensils (including knife) and Table clothes, cloth napkins, and centerpieces are in place.
- Assist MCD to ensure adaptive devices that have been recommended by OT & Speech, are utilized.
- In the Absence of the MCD, ensure Menus are posted or are available to residents for lunch and dinner.
- Assist MCD to ensure show plates are used for residents to assist them in making menu choices prior to ordering.
- Assist the MCD to ensure staff utilize diet book as a reference when serving meals.
- Assist the MCD to ensure all associates are wearing the DOH recommended grey barrio jackets during meals.
Family Education & Support
- Assist with providing a monthly family support group.
- Assist with provide frequent Communication with families including sending calendar newsletter, pictures, emails, and phone calls ; providing positive reassuring communication.
Education/Experience/Licensure/Certification:
- An associate degree from an accredited college or university or satisfactory completion of two years of college, with major work in social work, nursing, psychology, gerontology, or therapeutic recreation or related field, or two years of full-time experience in the recreation field with an adult population may be considered.
- Is a Certified Dementia Practitioner CDP® or will complete the necessary steps to become certified.
- Must possess a passion for working with seniors and improving their quality of life.
- Has a basic understanding of Alzheimer’s and other dementia-related diseases is essential.
- Excellent communication, organization and time management skills.
- Basic typing skills and computer knowledge; familiarity with MS Word, Excel, Publisher and creative software.
