Summary
SUMMARY: Provides structured orientation, mentoring and on-going annual and monthly education to provide systems level support to the following individuals and groups within the Care Coordination Department: new employees, existing staff, and social work interns with the goal of professional development to best serve patients with complex bio-psychosocial needs.
DUTIES & ESSENTIAL JOB FUNCTIONS: NOTE: Following are the duties performed by employees in this classification. However, employees may perform other related duties at an equivalent level. Not all duties listed are necessarily performed by each individual in the classification.Â
1. Coordinates with Department Managers regarding new staff onboarding needs with Care Coordination.
2. Schedules orientation with new staff and staff preceptors including but not limited to Electronic Health Records Training, Structured Orientation, Shadowing opportunities.
3. Conducts new staff orientation following departmental orientation structure and guideline.
4. Liaises with key staff and providers within the system to support orientation and collaboration.
5. Develops a series of brief educational sessions on care coordination related topics for resident, intern and attending onboarding
6. Provides training for social workers to provide evidence-based screening tools (e.g. suicide risk assessment) and on0going evaluation of clinician competence
7. Develops written orientation material as needed to aid learning.
8. Maintains and updates New Staff Orientation binders (electronically and hard copy) for distribution.
9. Develops annual continuing education plan for social workers under the direction of leadership. Facilitates on-site CEUs/certificate/license renewal opportunitiesÂ

10. Develops and maintains competency checklist for new and existing social workers
11. Partners with the Clinical education department to complete annual survey’s and documented learning needs assessments that inform the annual staff competency reviews
12. Assesses standards of practice for AHS social workers (in inpatient areas and emergency department) for compliance status, incorporates this information in monthly /annual trainings.Â
13. Recruits, coordinates, and hosts with in-house and community partners for training/presentation sessions (in-person and video-based)
14. Tracks and maintains records of training for trending,process improvement purposes and meeting license requirements
15. Supports clinical and workflow updates by organizing and executing training and refresher sessions (e.g., EPIC upgrades)
16. Provides elbow-support to staff to support learning of processes
17. Develops and maintains contractual and practice relationship with MSW Field Education Programs
18. Recruits and supports Field Instructors within AHS
19. Directs and supports the application, selection process for each cohort of MSW Interns with the Field Instructors
20. Collaborates with Human Resources on any training programs and onboarding process.
21. Conducts and coordinate structured orientation.
22. Troubleshoots and mediate logistical issues between interns, field instructors and school field liaisons
23. Supports staff upon request to consult on complex patient care issues, resource management and legal/ethical concerns.
24. Develops a pathway for MSW interns to transition to ALH associate social worker positions after successful completion of ALH internship
25. Provides group and individual clinical supervision for social workers working toward clinical licensurePerforms other duties as assigned.
MINIMUM QUALIFICATIONS:
Any combination of education and experience that would likely provide the required knowledge, skills and abilities as well as possession of any required licenses or certifications is qualifying.
Required Education: Master's degree in social work/welfare issued by a school accredited by the Counsel of Social Work Education.
Required Experience: Three years of Social Work or Case Management experience in a medical setting or community agency; experience in developing training curriculum, coordinating session logistics, and conducting small and large group trainings.
Preferred Experience: Experience in an acute care hospital setting.
Required Licenses/Certifications: Valid license as a Clinical Social Worker issued by the State of California Board of Behavior Science Examiners.
Preferred Licenses/Certifications: Certified Case Manager (CCM)
