The Terraces of Phoenix, a HumanGood community named one of the Best Nursing Homes of 2022-2023 by U.S. News and World Report is hiring a PRN/On-call Licensed Practical Nurse (LPN/LVN) for its Skilled Nursing Team.

** PRN/On-Call   ** Must have Availability for AM (6:30am-2pm) and Evening Shifts (2:30-10pm)   ** $32-$40/hr. (DOE)

** Shift Differentials: + $1.00/hr. for PM Shift; + $1.50/hr. for Night (NOC) Shift; + $2.00/hr. for PM Shift on Weekends;  + $2.50/hr. for Night (NOC) Shift on Weekends

Our Licensed Practical Nurses (LPN/LVN) are responsible for the nursing needs of residents, while actively incorporating our “Philosophy for person-directed Care.” You will ensure a safe and healthy environment for residents, staff, and visitors to participate in the resident’s daily life rhythm. You would help develop assignments in coordination with the care partners and keep each team member accountable for resident care and satisfaction.

To be successful in the role, you would have:

  • Current LPN/LVN in the state for which applying
  • Current CPR certification
  • Prior nursing experience: senior care, skilled nurse, post-acute or sub-acute care preferred
