Occupational Therapist - Home Health - (OT - HH)
StartDate: ASAP Available Shifts: 8 D Pay Rate: $2289.92 - $2509.16
How long is the contract assignment? 13 weeks is the norm: 13 weeks 4. Schedule of assignment hours? EX: M – F 8am – 5pm? M-F 8 am – 5pm 5. Will there be any weekend rotation? If so, how often? No weekends 6. Will there be any on call? If so, how much? No on call 7. What region are you in? Home health or Hospice? Home Health 8. What territory this contractor will be asked to cover? North East Valley – North Phoenix, Scottsdale, Tempe, Cave Creek, Carefree and Fountian Hills 9. What’s creating the need for a contractor? - Moving current FT OT to cover South area for growth. Currently declining referrals in far south east. 11. Which location will this person report to for their assignment- add info below Full E Missouri Ave Ste 265 Phoenix AZ 85012 12. About how many miles per week will the contractor travel? 300-400 13. What are the weekly units of productivity expectations? 30 points/wk 14. What EMR systems will the consultant be using: HCHB/Pointcare
