About The Position

We are seeking a skilled Family Practice Nurse Practitioner for a locum tenens position with a Home Health agency in Oregon. This 60-day assignment starts on 06/02/2025, with a flexible schedule ranging from 8:00 AM to 8:00 PM, with a minimum of 8:00 AM to 5:00 PM shifts available. The ideal candidate will be comfortable conducting Medicare Assessments, including reviewing medical history and medications, completing HRA/Charts, and performing required screenings such as Dexa, Retinal Screening, and FOTB. This role offers an opportunity to provide in-home patient assessments and screenings, ensuring comprehensive care for home health patients. The Nurse Practitioner will be responsible for conducting thorough assessments, documenting findings, and contributing to patient care plans.

Job Responsibilities and Duties:Ā 

ā€¢ Conduct comprehensive Medicare Assessments in patients' homesĀ 

ā€¢ Review and document medical history and current medicationsĀ 

ā€¢ Complete Health Risk Assessment (HRA) charts accuratelyĀ 

ā€¢ Perform required screenings, including Dexa, Retinal Screening, and FOTBĀ 

ā€¢ Receive training on specific screening procedures prior to conducting themĀ 

ā€¢ Utilize Focus Care EMR system for patient documentationĀ 

ā€¢ Collaborate with the home health team to ensure quality patient careĀ 

ā€¢ Provide patient education on health maintenance and disease preventionĀ 

ā€¢ Adhere to all agency policies and procedures for in-home assessmentsĀ 

ā€¢ Maintain accurate and timely medical recordsĀ 

ā€¢ Demonstrate flexibility in scheduling and travel within a 30-mile radius or greaterĀ 

ā€¢ Participate in any required training or orientation sessionsĀ 

ā€¢ Communicate effectively with patients, families, and healthcare team membersĀ 

ā€¢ Contribute to the development and implementation of patient care plansĀ 

ā€¢ Stay updated on best practices in home health assessments and screenings
