Valle Verde in Santa Barbara, recently named "Best Continuing Care Retirement Community in the U.S." by Newsweek magazine is hiring a Medical Social Worker. Under limited supervision, the Medical Social Worker assists post-acute patients and their families to navigate the skilled nursing rehabilitation experience.

** Annual Salary Range: $64,000 - $90,000   ** Monday - Friday, Flexible Hours Schedule

****  PLEASE NOTE:  For MSW's currently pursuing LCSW certification, Valle Verde does not have an LCSW or LPCC onsite to supervise/approve your hours.

Essential duties and responsibilities include, but are not limited to:

  • Ensuring that Health Care Center residents' social service needs are identified in the care plan, that they are documented in the medical records, and that appropriate action is taken
  • Developing/facilitating resident group activities, such as support, peer, grief and wellness groups with a therapeutic focus to meet the social and emotional needs residents
  • Visiting and interviewing residents on admission, at quarterly intervals, when requested by staff, physicians, residents or family members, and as needed; revises care plan as necessary
  • Ensuring a social service assessment is completed with accurate documentation for all new residents within 5 days in the Health Center or RCFE move-in
  • Overseeing discharge planning for residents of Health Care Center to their home or other location, coordinating with therapy, nursing, physicians, family, home health agencies, etc.
  • Monitoring Medicare eligibility period and provides required notices to residents/responsible parties;
  • Assisting Director of Admissions with preparing for prospective admissions, including contact with hospital discharge planners, tours for family/responsible party, coordinating with nursing team, etc.
  • Assisting Charge Nurse and Inter-Disciplinary Team in identifying specific problems/needs to be addressed on the resident care plan;
  • Attending in-service training workshops and meetings as required
  • Performing related essential duties as required

What you need to be successful in this role:

  • Masters of Social Work Degree
  • At least 3 years of experience working in healthcare setting, preferably with geriatric residents; or any equivalent combination of training and experience which provides the required skills, knowledge and abilities.
  • Managed care experience
  • Ability to interface effectively with residents, providers, management and team members of the organization as well as facilities and agencies in the community
  • Excellent assessment and decision making skills
