Serves in an advanced role providing specialized care and education for persons with diabetes and their families. Serves as a team member in assessing needs, planning and providing education and care, evaluating outcomes and coordinating follow up care for persons with diabetes. Job requires one of the following: Registered Nurse or Registered Dietitian Nutritionist or Doctor of Pharmacy. Evaluation of the individuals being seen for Diabetes Self-Management Education, Training, and Support knowledge of diabetes, diabetes self-management skills, support and barriers to care. Provides direct care to individuals with diabetes and their families in collaboration with the primary care physician and Endocrinologist. Assessing general diabetes knowledge, evaluate physical finding, anthropometric measurements, evaluate physical findings. Provide general - sick day information and basic foot care. Teach use of blood glucose meter and teach insulin preparation and injection skills and techniques. Develops basic diabetes management plans appropriate to patient's immediate diabetes needs. Reviews specific indicators measured and results, helps regulates diabetes metabolic control of persons with diabetes on an outpatient basis and provides phone contact for medication adjustments and problem-solving under the guidance of Physician, CRNP, Physician Assistant. Serves as a resource person for patient and family regarding the perception of the diagnosis and medical plan of care for the management of disease as designated by the physician. Investigates and problem solves patient care issues with staff and facilitates change to promote quality outcomes. Enhance quality of diabetes care through collaboration with other services and initiate referrals to appropriate support services. Works with in the multidisciplinary team for diabetes education and management and collaborates with other disciplines to identify and resolve patient care issues. Conducts diabetes education through group classes as needed. Provides and participates in community and in-house education programs and acts as the department and liaison in the community and for hospital staff as requested. Serves as communication link between the department and the hospital staff both inpatient and outpatient. Implements outcome-based quality assurance and performance improvement activity to document outcomes of service, compliance with regulations, policies and procedures and monitors and addresses customer satisfaction. Assists physician with research involving education and care of patients. Work is typically performed in a clinical environment.
