Salaried position: Range $80,000 to $83,200

The Medical Social Worker is responsible for providing clinical social work services to residents and families on an outpatient basis including counseling, crisis intervention, short-term case management, and information and referrals to community resources. The Social Worker organizes, implements, and evaluates the Social Services program to meet the psycho-social needs of residents in the Health Care Center; may work with residents in other levels of living when needed; will also assist providers and Care Managers by acting as a consultant for residents with complex psychosocial needs. As a representative and team member of the company, this position is expected to present oneself in a manner that reflects professionalism and ensures resident satisfaction.

- Education – Masters of Social Work Degree or Licensed Clinical Social Worker.

- Experience/Training – At least 3 years of experience working in healthcare setting, preferably with geriatric residents; or any equivalent combination of training and experience which provides the required skills, knowledge and abilities. Managed care experience.

- Certificates, Licenses, Registrations – MSW or LCSW preferred.
