Essential Functions
Performs direct patient care following an individualized plan of care, the plan of care includes assessment, intervention, including medication administration, specimen collection, clinical treatments, as well as other medical care treatment and re-evaluation of the patient. In the Pre-Operative and Post-Operative environment.
Documents patient care given in Med-Host, Nextgen and Exit Care (EMR systems).
Asses, monitors, records, and communicates patient condition as appropriate to care team, physician, patient, and family. Assures all pre-operative requirements are met prior to the beginning of the procedure.
Assists patients with performing activities of daily living, including personal hygiene, elimination, nutrition, and ambulation.
Collaborates as needed across disciplines to coordinate patient care, including patient transfer, discharge, referral and spiritual/psychosocial support needs.
Evaluates learning needs of patient and/or family. Provides patient/family education appropriate to age, culture, condition, and circumstances per physician orders.
Works as an advocate for the physical and emotional well-being of the patient.
