Job Summary

The Personal Care Professional is responsible for providing patients with in-home personal care and designated health related services to maintain the patient's physical and emotional well being, while following the written plan of care, Medicare/Medicaid regulations and agency policies and procedures.

Specific Job Duties/Responsibilities

  • Follows current written aide assignment sheet and provides care in accordance with the State Practice Act, including personal care and assistance with activities of daily living (ADLs) to include mobility transfer, walking, grooming, bathing, dressing or undressing, eating, or toileting. 
  • For the HCBS service line assignment may also include assistance with meal preparation, shopping/errands, transportation, and housekeeping tasks, and medication assistance/reminders. 
  • As assigned, performs incidental household services essential to the patient's health care at home that are necessary to prevent or postpone institutionalization.
  • Completes a clinical note for each visit within required timeframes, which must be incorporated into the patient's record. 
  • For the HCBS service line, utilizes the Electronic Visit Verification (EVV) program as applicable including documenting all client related activities and tasks timely.
  • Participates in daily report, participates in case conferences, and adheres to and supports the care management model within the agency.
  • Participates in the organization’s Quality and Compliance programs including any performance improvement plans, maintains confidentiality, and adheres to the organization’s policies and procedures.
  • Records and notifies RN/Patient Care or Clinical Manager/Agency Director of any variations from the established parameters or any changes in the patients condition.
  • Provides a clean and safe environment (including safe use of assistance equipment). Drives safely, complies with traffic regulations, and wears a seat belt when business travel is required.
  • Provides emotional support & compassionate care to the patient & family at all times.
  • Functions as a preceptor to new hires as needed and/or other duties as assigned.

 


Experience Desired
  • 1 year of experience in home care.
Skill Requirements
  • Successful completion of a competency evaluation.
  • Current driver's license, vehicle insurance, and access to a dependable vehicle or public transportation
  • Must be mature and able to deal effectively with the demands of a Multi-service line aide position. Able to read, write, and carry out directions promptly and accurately.

Additional State Specific Requirements

TX:  Must have a minimum of one-year full-time experience in direct client care in hospital or nursing facility, OR one year full- time experience within last 5 years in direct client care in agency setting, OR documented completion of training and competency evaluation program with home health experience within the last 24 months, OR documented completion of competency evaluation program with home health experience within the last 24 months, OR nursing student with documentation from director of programs or the dean of a school of nursing that states that the individual is a nursing student who has demonstrated competency in basic nursing skills aligned with the school's curriculum, OR be on Texas Department of Human Services (DHS's) Nurse Aide Registry with no findings of client abuse or neglect or misappropriation of client property. 

Must NOT be listed on the Employee Misconduct Registry, been revoked on the Nurse Aide Registry, or had a criminal conviction as listed in Health and Safety Code Chapter 250.
