Job Description:
Under the supervision of the Care Coordination and Resource Services Supervisor, the LVN is responsible for post discharge follow up calls to ensure discharge plan is followed. Assists with arranging appropriate follow up appointments as needed and reviews the after visit summary, including medication reconciliation. The LVN works in collaboration and in conjunction with the RN and Care Coordination and Resource Services Team to facilitate appropriate care and interventions. The LVN provides follow-up care as needed, as well as pertinent patient education, referral to relevant educational opportunities and information sources designed to maintain function, prevent illness, and promote wellness.Education
Graduate of accredited Licensed Vocational Nurse Program
Experience
Two (2) years of clinical experience working in a medical setting with a diverse patient population – Required.
One year of care coordination health plan or ambulatory – Independent Physician Practice (IPA) or Medical Group or case management experience – Preferred
Certifications/Licensures
LVN Licensed Vocational Nurse – California Board of Vocational Nursing - Required
BLS Basic Life Support - American Heart Association - Required
