Job Description:
The Social Worker for the Cancer Program performs Social Service duties for assigned patients in the program. Responsibilities include regular liaison with members of the treatment team, discharge planning, aftercare follow-up and liaison with community agencies as needed. The Social Worker conducts the psychosocial assessment for assigned patients. The Social Services Clinician provides support to the patients and their family systems. The Social Worker helps to coordinate and to provide family services. The Social Worker reports to the Manager of Oncology and Specialty Services.Education:
Master's Degree - Social Work - Required

Experience:
1 year of Social Work in a Hospital or Hospital Setting - Required
Certifications/Licensures:
BLS Basic Life Support - American Heart Association - Required
LCSW Licensed Clinical Social Worker - California Board of Behavioral Sciences - Preferred
CPI Non-Violent Crisis Intervention Training- Required within 30-days of hire and renewal annually or within 30-days of return from a leave of absence.

Must have computer skills for word processing, internet, and e-mail usage.
