Job Overview

This role is vital in supporting patient flow within the Frailty Same Day Emergency Care (FSDEC) unit. Responsibilities include same-day assessments, rehabilitation, re-enablement, and discharge planning to ensure seamless care for patients with complex needs.

Key Responsibilities
  • Perform same-day assessments and facilitate timely discharges within FSDEC.
  • Develop and execute individualized rehabilitation and re-enablement strategies.
  • Collaborate with multidisciplinary teams to optimize patient outcomes.
  • Conduct effective discharge planning to ensure continuity of care.
  • Maintain accurate and high-quality documentation per NHS standards.
Requirements
  • HCPC Registration as a Physiotherapist.
  • Demonstrated experience in patient flow, rehabilitation, and discharge planning.
