Job OverviewThis role is vital in supporting patient flow within the Frailty Same Day Emergency Care (FSDEC) unit. Responsibilities include same-day assessments, rehabilitation, re-enablement, and discharge planning to ensure seamless care for patients with complex needs.
Key Responsibilities- Perform same-day assessments and facilitate timely discharges within FSDEC.
- Develop and execute individualized rehabilitation and re-enablement strategies.
- Collaborate with multidisciplinary teams to optimize patient outcomes.
- Conduct effective discharge planning to ensure continuity of care.
- Maintain accurate and high-quality documentation per NHS standards.
- HCPC Registration as a Physiotherapist.
- Demonstrated experience in patient flow, rehabilitation, and discharge planning.
- Strong ability to handle a diverse and fast-paced caseload.
- Exceptional communication and organizational skills.
- Commitment to delivering excellent, patient-centered care.
