Ward: Barnet North Core Team

Pay Rates: 拢21.97 Per hour

Shift Patterns: Monday - Friday 9.00am - 17.00pm

Barnet, Enfield and Haringey Mental Health NHS Trust are currently looking for an experienced Care Coordinator to join their Barnet North Core Team at Springwell Centre. If you are looking to develop your NHS experience and provide specialist care, then please do apply!

This post is suitable of Community Mental health nurse, Social Worker or Occupational Therapist. Within this role, as a Band 6, you will have the responsibility of an allocated caseload of 25-30 service users. This will entail complex cases of patients with severe MH illness and who possibly require additional support around safeguarding and legal requirements. As a care coordinator you will lead on the treatment plan to ensure service user needs are met.

What you can add:

  • Act as key worker for individual caseloads
  • Promote knowledge of mental health and prevention of mental health illness
  • Plan and implement client centred induvial or group interventions to achieve therapeutic goals

What you鈥檒l need:

Degree or equivalent in OT, SW or MH Nursing

Experience of working within the NHS

Mental Health Experience

In addition to experience, all Qualified roles will be required to provide evidence of:
